    Dear Head of lab,

The British Heart Foundation and the National Familial Hypercholesterolaemia (FH) steering group have requested the help of the ACGS to measure the improvement in ascertainment of FH cases achieved from the current rounds of investment. Measuring the benefit of the programme is key to make an argument for sustained and hopefully extended commissioning for FH testing. The steering group understand the amount of work involved particularly in collecting retrospective data, but this is key to establishing a base line from which to measure improvement. Everyone's efforts will be greatly appreciated.

The ACGS Quality Subcommittee would like to collect information from laboratories provided FH testing, thereforewe invite you to complete the attached survey.

Please complete this electronic copy and email to Sandi Deans at Sandi.Deans@ed.ac.uk by 2nd October 2015.

    Thank you for your help.

    With best wishes,
    Sandi

     Dr Sandi Deans
